Your Email Address (Must be provided or we can not send you the attendance information)

Desired action - Tick Relevant Boxes (both if relevant)

I want to secure a "Booking for myself only"
I am "Booking for myself and additional attendees"
I am only "Booking for others".

List all Names, Addresses, Contact No's of those attending if you are booking for others , also list yourself if you want to attend.

Course Description and Date - You may place more than one course and date here ( Check the Calendar Tab for correct course dates)

Booking Conditions

It is clear and understood that when I request a booking with Active TEC that I have established a contract with Active TEC to provide a service. I agree I will be legally required to pay the fees, unless I have cancelled the booking with Active TEC with 7 days notice.
